Patient/Guarantor responsibilities:

It is the expectation of CSG that the guarantor will provide all necessary insurance information at the time of your scheduled appointment. This would include any required referrals and/or authorizations from your child’s primary care physician.

Guarantors will also be required to pay any cost shares at the time services are rendered.

Payment Options:

Children’s Specialty Group provides services to all children in need of care. We will assist all families in developing payment schedules based upon individual financial situations. Our financial counselors are available between the hours of 8 a.m. to 4:30 p.m., Monday through Friday. We are willing to assist parents and/or guardians to establish a mutually acceptable financial plan. Parents and/or guardians can make appointments to visit our business office or call (757) 668-7200 to speak with a financial counselor for assistance.
