Patient/Guarantor responsibilities:

It is the expectation of CSG that the guarantor will provide all necessary insurance information to their scheduled appointment. This would include a referral or authorization number from your child's primary care physician. The guarantor will also be responsible for ensuring that co-pays are paid at the time services are rendered.

Payment Options:

CSG provides services to all children in need of care. We will assist all families in developing payment schedules based upon individual financial situations. Our financial counselors are available between the hours of 8 a.m. to 4:30 p.m., Monday through Friday. CSG is willing to assist parents and/or guardians to establish a mutually acceptable financial plan. Parents and/or guardians may visit our business office or call (757) 668-7200 to speak with a financial counselor for assistance.
